There's a link in the description that leads to more details on the screenshot. There I've posted some answers that might help you set it up. Additionally, there is a photoshop template of this theme in my gallery on Deviantart.
These are the apps you need: ADW Launcher EX, Multipicture Live Wallpaper.
Optional apps: Minimal Text, MissedIt!
I can't really run through everything step by step because it's different for everyone's phone. I'm using a Sony Xperia P so the resolution is different from other phones. This is important when setting up Minimal text to display date, weather, etc. But if you understand the concept of this setup you can make it work for your phone. Like I said, try the link in the description. You will probably find some answers there.
